Welcome to my in-home childcare. I thank you for choosing me as your provider. I look forward to working with you as we work to provide the best care for your child. I’ll leave this with you as you register so you can refence if you need to. It is important these be all filled out prior to care or care cannot begin.
Parents Responsibilities
Sign contract after reviewing handbook
All papers issued signed and dated
Bring shot records before care can begin
Label all items brought to daycare with child’s name
Our Rates
$130.00 ($4.00 per hour per child once 50 hours has been reached)
$120.00 a week ($4.00 per hour per child once 25 hours has been reached)
$30.00 per day (Per child 9-hour max than $4.00 after the 9 hours has been reached)
$25.00 a day
$4.00 per hour (3-hour limit)
$1.00 per minute
$10.00 per day
Payment Information
** Checks will not be accepted unless agreed upon**
** No refunds for care**
**I do require full payment for my part-time and full-time children even if absent** (Including days I take off for sick or scheduled vacation) unless arrangements are made. **You are paying for your spot, not the service of days your child comes. **
Policies
Any changes or circumstances, which may affect my ability to comply with licensing rules such as but not limited to new program location, building renovations or remodeling, suspected in-house child abuse or neglect, ownership change, employee felony convictions or new director will be reported to Child Care Licensing within 24 hours of occurrence.
